| Summary of Position: The Integrative Hub is seeking a credentialed, mission-aligned Integrative Health Provider to join our care team as an independent contractor. This provider serves as the clinical anchor of a coordinated, non-prescribing Direct-to-Consumer clinic, working alongside our Health Coaching team. This is not a primary care or urgent care role; it is a focused, relationship-driven practice rooted in evidence-based lifestyle intervention. The provider conducts comprehensive lifestyle and health history intakes, orders and interprets standard lab panels (lipids, metabolic panel, HbA1c, thyroid, hormones, and related), and develops individualized treatment plans across the six pillars of Lifestyle Medicine: nutrition, physical activity, sleep, stress management, avoidance of risky substances, and positive social connection. Through follow-up visits, the provider assesses progress and adjusts plans, offering health education and motivational interviewing along the way. The provider also coordinates with patients' primary care providers and recommends referrals to specialists or other care settings as needed. Because this is a non-prescribing model, no Collaborative Practice Agreement is required under Indiana law, and the provider practices with full autonomy within this scope. At launch, the role does not include prescribing medications, urgent or emergent care, mental health diagnosis or psychiatric treatment, serving as a patient's primary care provider, ordering imaging, in-clinic procedures such as injections or IV therapy, or pediatric care, as the clinic serves adults 18 and older. Health Coaches receive clinical notes through our EMR (eClinicalWorks) and carry behavioral implementation forward, making clear documentation essential. The provider is engaged as a 1099 independent contractor, maintains active malpractice insurance, and manages scheduling in coordination with clinic availability.
