Department: Health Hub Enterprise - IWU-National & Global
Campus Location: Fort Wayne Education Center, Fort Wayne, IN
Summary of Position: The Integrative Hub is seeking a credentialed, mission-aligned Integrative Health Provider to join our care team as an independent contractor. This provider serves as the clinical anchor of a coordinated, non-prescribing Direct-to-Consumer clinic, working alongside our Health Coaching team. This is not a primary care or urgent care role; it is a focused, relationship-driven practice rooted in evidence-based lifestyle intervention. The provider conducts comprehensive lifestyle and health history intakes, orders and interprets standard lab panels (lipids, metabolic panel, HbA1c, thyroid, hormones, and related), and develops individualized treatment plans across the six pillars of Lifestyle Medicine: nutrition, physical activity, sleep, stress management, avoidance of risky substances, and positive social connection. Through follow-up visits, the provider assesses progress and adjusts plans, offering health education and motivational interviewing along the way. The provider also coordinates with patients' primary care providers and recommends referrals to specialists or other care settings as needed. Because this is a non-prescribing model, no Collaborative Practice Agreement is required under Indiana law, and the provider practices with full autonomy within this scope. At launch, the role does not include prescribing medications, urgent or emergent care, mental health diagnosis or psychiatric treatment, serving as a patient's primary care provider, ordering imaging, in-clinic procedures such as injections or IV therapy, or pediatric care, as the clinic serves adults 18 and older. Health Coaches receive clinical notes through our EMR (eClinicalWorks) and carry behavioral implementation forward, making clear documentation essential. The provider is engaged as a 1099 independent contractor, maintains active malpractice insurance, and manages scheduling in coordination with clinic availability.
Duties and Responsibilities
Conduct all clinical intake, lab review, treatment plan, and follow-up visits via EClinicalWorks (ECW)
Develop clear, actionable care plans that can be effectively carried forward by Health Coaches in behavioral coaching sessions
Maintain timely and thorough clinical documentation in ECW following each patient encounter
Communicate with patients’ primary care providers as clinically appropriate
Participate in regular care coordination with Health Coaches to support continuity across the clinical and coaching relationship
Maintain active professional liability insurance (reimbursed by IWU as part of the contractor arrangement)
Sign and adhere to a HIPAA confidentiality agreement as part of the contractor onboarding process
Operate within the defined non-prescribing lifestyle medicine scope without exception
Engage in ongoing professional development in lifestyle medicine and integrative health best practices
Qualifications: According to Indiana Wesleyan University employment policy all employees must possess a strong Christian commitment and adhere to the standards outlined in the IWU Community Lifestyle Statement.
Education
Active Indiana licensure as a Nurse Practitioner (NP), Doctor of Nursing Practice (DNP), or equivalent advanced practice clinician required
Board certification in a relevant clinical specialty required; Lifestyle Medicine certification (DipABLM or equivalent) strongly preferred
Experience
Experience practicing in a lifestyle medicine, integrative health, functional medicine, or preventive care setting preferred
Demonstrated ability to order and interpret standard clinical lab panels independently
Required Skills
Strong interpersonal and patient communication skills with a patient-centered, collaborative practice style
Comfort operating within a coordinated care model alongside non-clinical Health Coaches
Familiarity with EClinicalWorks or comparable EMR platforms preferred
Alignment with the mission and values of Indiana Wesleyan University
